摘要:
目的 探討超聲波對(duì)兔膝關(guān)節(jié)骨關(guān)節(jié)炎(knee osteoarthritis,KOA)關(guān)節(jié)滑膜和關(guān)節(jié)滑液血管內(nèi)皮生長(zhǎng)因子(vascular endothelial growth factor,VEGF)、基質(zhì)金屬蛋白酶-13(matrix metalloproteinases-13, MMP-13)表達(dá)的影響。方法 選取24只新西蘭大白兔隨機(jī)分為假手術(shù)組(sham-operated group,SO組)、骨關(guān)節(jié)炎組(osteoarthritis group,OA組)和超聲治療組(ultrasound treatment group,UT組)。采用前交叉韌帶橫斷術(shù)(anterior cruciate ligament transection,ACLT)法造模,UT組予超聲波治療儀治療兔膝關(guān)節(jié); OA組處理同上,但未打開(kāi)超聲波治療儀; SO組未做處理。干預(yù)2周后,抽取關(guān)節(jié)液標(biāo)本,并觀察標(biāo)本滑膜、軟骨的大體形態(tài); 酶聯(lián)免疫吸附法檢測(cè)關(guān)節(jié)滑液VEGF和MMP-13的表達(dá); Western blot方法檢測(cè)關(guān)節(jié)滑膜VEGF和MMP-13的表達(dá)。結(jié)果 與SO組相比,OA組和UT組兔膝關(guān)節(jié)骨關(guān)節(jié)炎評(píng)分均顯著升高(P<0.05); 與OA組相比,UT組兔膝關(guān)節(jié)骨關(guān)節(jié)炎評(píng)分顯著降低(P<0.05)。與SO組相比,OA組和UT組兔膝關(guān)節(jié)滑液及滑膜中VEGF和MMP-13的蛋白水平均顯著升高(P<0.05); 與OA組相比,UT組兔膝關(guān)節(jié)滑液及滑膜中VEGF和MMP-13的蛋白水平顯著降低(P<0.05)。結(jié)論 超聲波治療能明顯抑制兔膝關(guān)節(jié)滑膜增生,有效抑制關(guān)節(jié)滑液和滑膜組織中VEGF、MMP-13的表達(dá)水平,從而達(dá)到防治KOA的作用。
Abstract:
Objective To investigate the influence of ultrasound on the expression of VEGF and MMP-13 in the joint synovium and synovial fluid of rabbit with knee osteoarthritis(KOA). Methods Twenty-four New Zealand white rabbits were randomly assigned into three groups: the sham-operated group(SO group), the osteoarthritis group(OA group), and the ultrasound treatment group(UT group).The model was made by anterior cruciate ligament transection. The UT group was treated with an ultrasound therapy device on the rabbit knee joint; the OA group received the same treatment but the ultrasound therapy device was not activated; the SO group received no treatment. After two weeks of intervention, joint fluid specimens were extracted, and the gross morphology of the synovial membrane and cartilage of the specimens was observed. The expression of VEGF and MMP-13 in the joint fluid was detected by enzyme-linked immunosorbent assay(ELISA), and the expression of VEGF and MMP-13 in the joint synovial membrane was detected by Western blot method. Results Compared with the SO group, the OA and UT groups showed significantly increased osteoarthritis markers in the rabbit knee joints(P<0.05); compared with the OA group, the UT group showed significantly decreased osteoarthritis markers in the rabbit knee joints(P<0.05). Compared with the SO group, the OA and UT groups showed significantly increased VEGF and MMP-13 protein levels in the rabbit knee joint synovial fluid and synovium(P<0.05); compared with the OA group, the UT group showed significantly decreased VEGF and MMP-13 protein levels in the rabbit knee joint synovial fluid and synovium(P<0.05). Conclusion Ultrasound therapy can significantly inhibit the proliferation of synovial membrane in rabbit knee joints, slow down the progression of osteoarthritis, effectively reduce the expression levels of VEGF and MMP-13 in synovial fluid and synovial tissue, and thus achieve the goal of preventing and treating KOA.
