摘要:
目的 探討急性硬膜下血腫(ASDH)后顱內(nèi)靜脈充盈延遲(DIVF)與疾病預(yù)后的關(guān)系。方法 選擇2018年至2023年福州市長樂區(qū)人民醫(yī)院明確診斷為ASDH的患者105例,收集患者的癥狀、既往史、血小板計(jì)數(shù)、血腫量以及治療方式等資料。患者均行顱腦CT靜脈造影(CTV)檢查,觀察大腦內(nèi)靜脈和橫竇的顯影情況,評(píng)估是否有DIVF。采用logistic回歸模型分析ASDH患者預(yù)后的影響因素,探討ASDH后DIVF與患者預(yù)后的關(guān)系。結(jié)果 多因素logistic回歸分析提示,DIVF[OR=390.407,95%CI為(39.199, 3 888.287),P<0.001]和腦中線偏移[OR=7.587,95%CI為(1.174,49.042),P=0.002]是預(yù)后的影響因素。結(jié)論 DIVF和腦中線偏移是ASDH患者預(yù)后的危險(xiǎn)因素,具有臨床指導(dǎo)價(jià)值。
Abstract:
Objective To explore the factors related to the prognosis of patients with acute subdural hematoma(ASDH)and provide reference for the development of diagnosis and treatment plans for this disease. Methods A prospective study was conducted on 105 patients diagnosed with ASDH in Fuzhou Changle District People's Hospital from 2018 to 2023.Information on symptoms, medical history, platelet count, hematoma volume, and treatment methods after admission were collected, and intracranial veins were reconstructed to evaluate whether there was delayed intracranial venous filling(DIVF).The differences were compared in the above indicators between patients with good prognosis and those with poor prognosis.Results Multivariate logistic analysis suggested that DIVF[OR=390.407,95%CI:(39.199,3 888.287),P<0.001] and midline deviation[OR=7.587,95%CI:(1.174,49.042),P=0.002] was independent risk factors for poor prognosis.Conclusion DIVF and midline deviation are independent risk factors for poor prognosis in ASDH patients, which can be used to predict the prognosis of ASDH patients and guide clinical decision-making.
