摘要:
目的 探討原發(fā)性中樞神經(jīng)系統(tǒng)淋巴瘤(PCNSL)與脫髓鞘假瘤(DPTs)磁共振(MRI)的影像表現(xiàn),提高兩組疾病診斷準確性。方法 回顧性分析41例中樞神經(jīng)系統(tǒng)淋巴瘤和30例脫髓鞘假瘤的MRI檢查,并分析兩組MRI的影像學(xué)特征。結(jié)果 PCNSL組與DPTs組在年齡、性別和臨床癥狀上的差異無統(tǒng)計學(xué)意義(P>0.05),在病灶最大徑、彌散加權(quán)成像(diffusion weighted imaging,DWI)、表觀擴散系數(shù)(appearance diffusion coefficient,ADC)、磁敏感加權(quán)成像(magnetic susceptibility-weighted imaging,SWI)上存在統(tǒng)計學(xué)差異(P<0.05); PCNSL增強掃描呈明顯均勻強化,部分病灶出現(xiàn)典型“缺口征”、“尖角征”,磁共振波譜分析(magnetic resonance spectroscopy,MRS)出現(xiàn)凋亡相關(guān)寬大Lip峰,可見Lac峰; DPTs增強掃描呈邊緣環(huán)形輕度強化或無強化,MRS無特異性。結(jié)論 PCNSL組的彌散受限、SWI信號減低發(fā)生率高于DPTs患者,結(jié)合增強掃描和MRS,可提高診斷準確性。
Abstract:
Objective To investigate the imaging findings of magnetic resonance imaging(MRI)in the diagnosis of demyelinating pseudotumor and lymphoma of central nervous system,and to improve the correct diagnosis rate of the disease. Methods A retrospective analysis of 41 patients with central nervous system lymphoma and 30 patients with demyelinating pseudotumor was carried out. MRI was performed on these patients and the imaging features were summarized. Results There was no statistically significant difference in age,sex,and clinical symptoms(P>0.05). The MRI findings that showed statistical significance between the two groups included the maximum diameter,diffusion weighted imaging(DWI)combined with apparent diffusion coefficient(ADC), sensitive weighted imaging(SWI). Primary central nervous system lymphoma: MRI imaging features showed uniform enhancement characteristics after scan enhancement, manifested as “incision” and “angular”. And MRS showed highly elevated lipid peak and inverted lactic acid peak. Demyelinating pseudotumors: The lesions had round or no enhancement on MRI scan. And MRS showed no specificity. Conclusion The PCNSL group had a higher incidence of diffusion-limited and reducing SWI signal than patients with DPTs, which is of great value in improving the diagnostic accuracy of PCNSL and DPTs. Enhanced scanning and MRS also have diagnostic value in the two groups.
