目的 探討前纖維蛋白-1(Profilin-1)濃度與冠心病患者經(jīng)皮冠狀動脈介入治療(percutaneous coronary intervention,PCI)術后2年發(fā)生心血管事件的相關性。方法 本研究納入772例冠心病患者PCI術后患者進行為期2年隨訪。根據(jù)患者是否發(fā)生不良心血管事件,將患者分為A組(67例發(fā)生不良心血管事件)和B組(705例未發(fā)生不良心血管事件)。同時評估Profilin-1對PCI術后的患者2年內(nèi)發(fā)生不良心血管事件的預測價值。結(jié)果 冠心病患者隨訪2年發(fā)生心血管事件率與Profilin-1濃度,與B組比較,A組Profilin-1更高[133.85(110.09～157.55)ng/mL],差異均有統(tǒng)計學意義(P<0.01)。對PCI術后患者2年內(nèi)并發(fā)不良心血管事件的預測價值中,ROC曲線下面積為0.821[95%可信區(qū)間(0.762,0.81),P<0.01]。并且ROC曲線分析中建立的臨界值基礎上,將血清Profilin-1濃度分為高危組(≥120.205 ng/mL)和低危組(<120.205 ng/mL)。根據(jù)Kaplan-Meier生存曲線所示,血清Profilin-1濃度高危組較低危組的患者心血管事件的發(fā)生率明顯升高。結(jié)論 Profilin-1濃度在一定程度上可預測2年心血管事件發(fā)生。
Abstract:
Objective To investigate the levels of profilin-1 concentrations associated with 2-year major adverse cardiovascular events in patients after percutaneous coronary intervention.Methods A total of 772 patients who had been followed up for two years were enrolled in this study.Depending on whether the patients had adverse cardiovascular events, the patients were divided into Group A(67 cases with adverse ardiovascular events)and Group B(705 cases without adverse cardiovascular events).Profilin-1's predictive value was assessed in detecting 2-year major adverse cardiovascular events in patients after percutaneous coronary intervention.Results Compared with those without adverse cardiovascular events, patients with coronary heart disease who have been followed up for two years were detected higher levels of profilin-1 [133.85(110.09-157.55)ng/mL,P<0.01].For 2-year major adverse cardiovascular events in patients after percutaneous coronary intervention, the area under the curve was 0.786 by using ROC curve in predicting MACEs [95%CI(0.724-0.848), P<0.01)].ROC curve analysis was used to evaluate the optimal cut off values. Serums profilin-1 concentration was categorized into high(≥120.205 ng/mL)and low(<120.205 ng/mL)groups.According to the Kaplan-Meier survival curve, the incidence of cardiovascular events in patients with high risk group with serum profilin-1 concentration was significantly increased than those with low risk group.Conclusion The profilin-1 concentrations may predict 2-year major adverse cardiovascular events in patients after percutaneous coronary intervention.
