摘要:
目的 探討幽門螺桿菌(Hp)感染與非酒精性脂肪性肝病(NAFLD)的關(guān)系。方法 收集我院行快速尿素酶試驗同時行腹部超聲檢查的住院患者共920例,根據(jù)有無合并NAFLD,分為NAFLD組及對照組,通過二元logistic回歸分析Hp感染與NAFLD的關(guān)系。結(jié)果 總研究對象中,NAFLD組(387例)Hp陽性者為98例,對照組(533例)Hp陽性者為119例,NAFLD組Hp感染率較對照組高(25.3% vs 22.3%),但兩組Hp感染率差異無統(tǒng)計學意義(P=0.291); 但在DM人群中,NAFLD組(94例)Hp陽性者27例,對照組(61例)Hp陽性者9例,NAFLD組較對照組Hp感染率高(28.7% vs 14.8%),且兩組差異有統(tǒng)計學意義(χ2=4.048,P=0.048)。在調(diào)整除TC、LDL-C、HDL-C等3個血脂指標之外的其他因素后,二元logistic回歸分析仍顯示兩組Hp感染率差異有統(tǒng)計學意義(P=0.030)。而在調(diào)整上述3個血脂指標后,兩組Hp感染率差異無統(tǒng)計學意義(P=0.215)。另該3個血脂指標中只發(fā)現(xiàn)LDL-C與NAFLD存在關(guān)聯(lián)(P=0.000)。結(jié)論 DM患者Hp感染與NAFLD存在關(guān)聯(lián),且兩者之間的關(guān)聯(lián)可能是通過LDL-C介導。
Abstract:
Objective To investigate the relationship between Helicobacter pylori(Hp)infection and Nonalcoholic fatty liver disease(NAFLD). Methods A total of 920 inpatients who underwent rapid urease test and abdominal ultrasonography were collected.According to the presence or absence of NAFLD, they were divided into NAFLD group and control group.Logistic regression analysis was used to study the relationship between Hp infection and NAFLD.Results Among the total study subjects, 98 cases were Hp positive in the NAFLD group(387 cases), and 119 cases were Hp positive in the control group(533 cases).There was no significant difference in the Hp infection rate between the two groups(P=0.291); but in the DM population, 27 cases were Hp positive in the NAFLD group(94 cases), and 9 cases were Hp positive in the control group(61 cases).The infection rate of H.pylori in the NAFLD group was higher than that in the control group(28.7% vs 14.8%), and the difference between the two groups was statistically significant(χ2=4.048,P=0.048).After adjusting for other factors except cholesterol(TC), low-density lipoprotein cholesterol(LDL-C), high-density lipoprotein cholesterol(HDL-C), the Hp infection rate between the two groups was statistically significant(P=0.030).After adjusting the above three blood lipid indexes, there was no statistical significance in the difference of Hp infection rate between the two groups(P=0.215).In the other three blood lipid indexes, only LDL-C and NAFLD were found to be correlated(P=0.000). Conclusion There is an association between Hp infection and NAFLD in DM patients, and the association between the two may be mediated by LDL-C.
