摘要:
目的 研究環(huán)境內(nèi)分泌干擾物對(duì)子宮內(nèi)膜異位癥疼痛的影響,探討相關(guān)發(fā)病機(jī)制。方 法 采用氣相色譜質(zhì)譜聯(lián)用技術(shù)質(zhì)譜法檢測(cè)子宮內(nèi)膜異位癥患者和正常對(duì)照者尿液中鄰苯二甲酸 酯(PAEs)9種代謝物含量,量化分析子宮內(nèi)膜異位癥患者痛經(jīng)情況,研究代謝物和痛經(jīng)的相關(guān)性。 通過合并痛經(jīng)子宮內(nèi)膜異位癥患者基因芯片篩選疼痛相關(guān)通路和關(guān)鍵蛋白(GSE23339)。結(jié)果 子 宮內(nèi)膜異位癥患者尿液中鄰苯二甲酸-單-乙基己基酯(MEHP)、鄰苯二甲酸單甲酯(MMP)含量均明 顯高于正常對(duì)照(5.67±2.01)μg /L vs(2.54±0.85)μg /L和 (17.11±8.37)μg /L vs(6.67±2.01)μg /L,均P<0.001。MEHP與 患者痛經(jīng)VAS評(píng)分呈正相關(guān)(r=0.86,P<0.001)。基因測(cè)序分析表明子宮內(nèi)膜 異位癥并發(fā)疼痛患者體內(nèi)補(bǔ)體和凝固級(jí)聯(lián)通路明顯激活,重要調(diào)節(jié)蛋白有人補(bǔ)體成分1S(C1S)、 補(bǔ)體成分7(C7)、補(bǔ)體因子H(CFH)和蛋白S1(PROS1)。結(jié)論 PAEs與子宮內(nèi)膜異位癥發(fā)病有關(guān),可 能通過補(bǔ)體和凝固級(jí)聯(lián)通路介導(dǎo)子宮內(nèi)膜異位癥相關(guān)疼痛。
Abstract:
Objective To study the effects of endocrine disruptors on endometriosis-related pain and explore the pathogenesis of endometriosis-related pain.Methods Nine metabolites of phthalic acid esters(PAEs)in urine of patients with endometriosis and normal controls were detected by mass spectrometry.The dysmenorrhea of endometriosis patients was quantitatively analyzed, and the correlation between metabolites and dysmenorrhea was studied.The pain-related pathways and key proteins were screened by gene microarray in endometriosis who complicated with dysmenorrhea(GSE23339).Results The concentration of MEHP and MMP in the urine of endometriosis were significantly higher than those of the normal control [(5.67±2.01)μg/L vs(2.54±0.85)μg/L and(17.11±8.37) μg/L vs(6.67±2.01)μg/L,(P<0.001, respectively)].MEHP was positively correlated with VAS score of dysmenorrhea.Gene sequencing analysis showed that complement and coagulation cascades were activated in patients with endometriosis complicated with pain, and the important regulatory proteins were C1S, C7, CFH and PROS1.Conclusion PAEs are associated with the pathogenesis of endometriosis and may mediate endometriosis-related pain through complement and coagulation cascade pathways.
