摘要:
目的 探討多項腫瘤標記物在胃癌診斷中的篩選及應用價值。方法 通過 Array- ELISA檢測胃癌患者(胃癌組)、健康體檢者(對照組)血清中在癌胚抗原(CEA)、糖類抗原199 (CA199)、糖類抗原242(CA242)、糖類抗原724(CA724)、糖類抗原50(CA50)、甲胎蛋白(AFP)、 糖類抗原125(CA125)、糖類抗原153(CA153)、人絨毛膜促性腺激素(HCG)、糖分解烯醇酶(NSE) 這10種腫瘤標志物的水平,并考察這些單項腫瘤標志物與多項聯(lián)合檢測胃癌的敏感度、特異度 和準確度。結(jié)果 胃癌組除HCG之外,其余9種腫瘤標志物水平較對照組均明顯升高 (P<0.05)。單項檢測靈敏度、特異度及準確度最高的前三位腫瘤標記物,共計4種, 分別為CA724、CA199、CEA及AFP。CA724+CA199+CEA+AFP四聯(lián)聯(lián)合檢測的靈敏度、特異度及準 確度最高,分別為96.50%、97.94%、91.11%。結(jié)論 采用Array-ELISA聯(lián)合檢測CA724、CA199、 CEA及AFP將大大提升胃癌的早期診斷水平。
Abstract:
Objective To explore the value of screening and detection of single and multiple combined tumor markers in the diagnosis of gastric cancer.Methods Array-ELISA was used to detect the level of the substance of ten tumor markers,including carcinoembryonic antigen(CEA), carbohydrate antigen 199(CA199), carbohydrate antigen 242(CA242), carbohydrate antigen 724(CA724), carbohydrate antigen 50(CA50), alpha-fetoprotein(AFP), carbohydrate antigen 125(CA125), carbohydrate antigen 153(CA153), human chorionic gonadotropin(HCG), and saccharolytic enolase(NSE)in the serum of patients with gastric cancer and healthy subjects.The sensitivity, specificity and accuracy of these single tumor markers combined with multiple detection of gastric cancer were compared.Results In addition to HCG, the levels of the other nine tumor markers in the gastric cancer group were significantly higher than those in the control group (P<0.05).The top three single tumor markers with the highest sensitivity, specificity and accuracy were CA724,CA199,CEA and AFP.The sensitivity, specificity and accuracy of CA724+CA199+CEA+AFP was highest, which were 96.50%, 97.94%,91.11% respectively.Conclusion The combination of Array-ELISA for detection of CA724, CA199, CEA and AFP will improve the early diagnosis of gastric cancer greatly.
