摘要:
目的 探討外周血宏基因組二代測序(mNGS)技術(shù)對惡性血液病合并發(fā)熱患者的診斷價值。方法 回顧性分析2021年6月至2023年6月在我院住院治療的惡性血液病合并發(fā)熱的患者23份外周血mNGS結(jié)果,與傳統(tǒng)血培養(yǎng)檢測結(jié)果進行比較,分析其病原譜特征及抗感染的療效。結(jié)果 23例患者中,通過外周血mNGS技術(shù)檢出9例病毒感染、4例細菌感染、2例真菌感染、3例病毒和細菌混合型感染、1例病毒和真菌混合型感染。傳統(tǒng)外周血培養(yǎng)僅檢出3例細菌感染,主要為肺炎克雷伯菌及鮑曼不動桿菌,未檢出真菌及病毒。外周血mNGS陽性檢出率明顯高于傳統(tǒng)血培養(yǎng)(82.6% vs.13.0%, P<0.05)。根據(jù)外周血mNGS檢測結(jié)果進行針對性抗感染治療,有效率明顯優(yōu)于傳統(tǒng)經(jīng)驗性抗感染治療(73.6% vs.25.0%, P<0.05)。結(jié)論 外周血mNGS技術(shù)在惡性血液病合并發(fā)熱患者的病原學(xué)診斷上較傳統(tǒng)血培養(yǎng)具有更高的價值,有助于提高臨床抗感染的療效。
Abstract:
Objective To investigate the diagnostic value of peripheral blood metagenomic next-generation sequencing(mNGS)in patients with malignant hematologic diseases complicated with fever.Methods Twenty-three peripheral blood mNGS results from patients with malignant hematologic diseases complicated with fever who were hospitalized in Fujian Provincial Hospital from June 2021 to June 2023 were restrospectively analyzed, and compared with traditional blood culture tests.The characteristics of the pathogenic spectrum and the efficacy of anti-infection were analyzed.Results Among the 23 patients detected by peripheral blood mNGS technology, 9 cases were diagnosed of viral infection, 4 cases of bacterial infection, 2 cases of fungal infection, 3 cases of mixed viral and bacterial infection, and 1 case of mixed viral and fungal infection.Only three cases detected in traditional peripheral blood culture were diagnosed of bacterial infection, mainly Klebsiella pneumoniae and Acinetobacter baumannii, but no fungi and viruses were detected.The positive detection rate of mNGS in peripheral blood was significantly higher than that in traditional blood culture(82.6% vs.13.0%, P<0.05).The effective rate of targeted anti-infective therapy based on the results of peripheral blood mNGS was significantly higher than that of traditional empirical anti-infective therapy(73.6% vs.25.0%, P<0.05). Conclusion Compared with traditional blood culture, peripheral blood mNGS technology has higher positive detection rate than traditional blood culture in the etiological diagnosis of patients with malignant hematologic diseases complicated with fever, which is helpful to improve the clinical anti-infection efficacy.
