摘要:
目的 對(duì)腰椎終板Modic改變相鄰結(jié)構(gòu)的磁共振(MR)脂肪定量進(jìn)行比較。方法(1)以終板Modic改變發(fā)生率最高的L5~S1椎間盤為研究對(duì)象,收集腰椎終板Modic改變患者55名,在常規(guī)腰椎MRI掃描后加用脂肪分析計(jì)算技術(shù)(FACT)序列掃描;(2)測(cè)量各終板Modic改變上椎體脂肪分?jǐn)?shù)面積加權(quán)值(SFFaw)、下椎體脂肪比面積加權(quán)值(IFFaw)、鄰近雙側(cè)腰大肌和多裂肌的脂肪分?jǐn)?shù)面積加權(quán)值(FFaw)、髓核T2信號(hào)強(qiáng)度(SI)值及Pfirrmann評(píng)分;(3)比較L5~S1各終板Modic改變的SFFaw、IFFaw、腰大肌和多裂肌的FFaw及Pfirrmann評(píng)分的差異;(4)分析L5~S1椎間盤Modic1型、2型的SFFaw、IFFaw、腰大肌和多裂肌的FFaw與髓核T2信號(hào)SI值的相關(guān)性;(5)分析L5~S1椎間盤所有終板Modic改變的SFFaw、IFFaw、腰大肌FFaw、多裂肌FFaw與Pfirrmann評(píng)分值的相關(guān)性。結(jié)果(1)L5~S1椎間盤Modic 1、Modic 2、Modic 3的SFFaw、IFFaw、多裂肌FFaw、腰大肌FFaw、髓核T2信號(hào)SI值、Pfirrmann評(píng)分值的差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。(2)L5~S1椎間盤Modic 1、Modic 2的SFFaw、IFFaw、多裂肌FFaw、腰大肌FFaw值與髓核T2信號(hào)SI值呈Spearman負(fù)相關(guān),P<0.05,有統(tǒng)計(jì)學(xué)意義;(3)L5~S1椎間盤所有終板Modic改變的SFFaw、IFFaw、腰大肌FFaw、多裂肌FFaw與椎間盤的Pfirrmann評(píng)分呈正相關(guān),P<0.05,有統(tǒng)計(jì)學(xué)意義。結(jié)論 L5~S1椎間盤各終板Modic改變相鄰結(jié)構(gòu)的脂肪含量比較有差異,并與椎間盤的退變存在一定的相關(guān)性。
Abstract:
Objective To compare fat quantification in adjacent structures of lumbar endplate Modic change. Methods The L5-S1 intervertebral disc with the highest incidence of Modic was taken as the research object, 55 patients with endplate Modic change were collected, and the FACT sequence scan was added after the routine lumbar MRI scan; The area-weighted value of fat fraction in the upper vertebral body(SFFaw), the area-weighted value of fat ratio in the lower vertebral body(IFFaw), the area-weighted value of fat fraction in the adjacent bilateral psoas major muscle and multifidus(FFaw), the T2 signal SI value of the nucleus pulposus and the Pfirrmann score were measured; The differences in SFFaw, IFFaw, FFaw of psoas major muscle and multifidus, and Pfirrmann score of each Modic of L5-S1 intervertebral disc were compared; The correlations between SFFaw, IFFaw, FFaw of psoas major muscle and multifidus of Modic1 and 2 of L5-S1 intervertebral disc and the T2 signal SI value of the nucleus pulposus were analyzed; The correlations between SFFaw, IFFaw, FFaw of psoas major muscle and multifidus of all endplate Modic change of L5-S1 intervertebral disc and the Pfirrmann score were analyzed. Results There were statistically significant differences in SFFaw, IFFaw, FFaw of multifidus, FFaw of psoas major, T2 signal SI value of nucleus pulposus, and Pfirrmann score among Modic 1, Modic 2, and Modic 3 of L5-S1 intervertebral disc(P<0.05). The values of SFFaw, IFFaw, FFaw of multifidus, and FFaw of psoas major in Modic 1 and Modic 2 of L5-S1 intervertebral disc were negatively correlated with the T2 signal SI value of the nucleus pulposus(P<0.05), which was statistically significant. The SFFaw, IFFaw, FFaw of psoas major, and FFaw of multifidus of all endplate Modic change of L5-S1 intervertebral disc were positively correlated with the Pfirrmann score of the intervertebral disc(P<0.05). Conclusion There are differences in the fat content of adjacent structures of each endplate Modic change of L5-S1 intervertebral disc, and there is certain correlation with the degeneration of the intervertebral disc.
