摘要:
目的 探討在大鼠器官獲取模型的灌注環(huán)節(jié),利用加入川穹嗪的乳酸鈉林格灌注液替代高滲枸櫞酸腺嘌呤液(HC-A液),對后續(xù)靜態(tài)冷保存腎臟質(zhì)量的影響。方法 建立大鼠腎臟獲取模型,根據(jù)灌注液成分不同分為乳酸鈉林格液(LRS)組、加入川芎嗪的乳酸鈉林格液(TMP-LRS)組、高滲枸櫞酸腺嘌呤液(HC-A)組,大鼠腎臟取下后進(jìn)行靜態(tài)冷保存(0~4℃)。根據(jù)保存時間階段分為不同組:腎臟(0 h、6 h、12 h、24 h、36 h),利用光鏡觀察腎臟組織的病理改變,TUNEL法檢測細(xì)胞凋亡,并檢測細(xì)胞ATP酶活性。結(jié)果 在靜態(tài)冷保存過程的6 h及后續(xù)時間點(diǎn)檢測腎臟組織ATP酶活力,與HC-A與TMP-LRS組相比,LRS組最顯著(P<0.01),各時間點(diǎn)TMP-LRS組與HC-A組相比差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。腎臟組織光鏡下病理改變主要表現(xiàn)為腎小管的腫脹、腎小管細(xì)胞壞死脫落、間質(zhì)水腫、血管內(nèi)皮細(xì)胞的壞死,24 h后變化明顯,LRS組變化最顯著。腎臟組織保存24 h時間點(diǎn)3組差異明顯,灌注保存36 h后,TMP-LRS組凋亡水平較HC-A組低,但差異無統(tǒng)計(jì)學(xué)意義。結(jié)論 利用加入川穹嗪的乳酸鈉林格液器官獲取時進(jìn)行灌洗,后續(xù)進(jìn)行腎臟保存可取得與HC-A液相當(dāng)?shù)男Ч?可作為替代以降低醫(yī)療成本。
Abstract:
Objective To investigate the effects of Lactate Ringer's solution with Tetramethylpyrazine as a substitute for hypertonic citrate adenine solution(HC-A solution)in the perfusion phase of rat organ acquisition model on the quality of kidney after static cold preservation.Methods An acquisition model was established, and the rats were divided into three groups according to the perfusion solution: Lactate Ringer's solution(LRS)group, Lactate Ringer's solution with Tetramethylpyrazine(TMP-LRS)group, and hypertonic citrate adenine solution(HC-A)group.After the liver and kidney were removed, they were subjected to static cold storage(0-4 ℃).The groups were based on the storage time: kidney(0 h, 6 h, 12 h, 24 h, 36 h).Pathological changes in organ tissues were observed by using photoscopic examination, cell apoptosis was detected by using TUNEL method, and cell ATPase activity was detected.Results During static cold storage, kidney ATPase activity gradually decreased, with LRS group showing the most significant decrease compared to TMP-LRS and HC-A groups at 6 h and later time points(P<0.01).There was no significant difference between TMP-LRS and HC-A groups at all time points(P>0.05).The kidneys showed mainly tubular swelling, tubular cell necrosis and shedding, interstitial edema, and endothelial cell necrosis, with changes becoming more obvious after 24 hours, and the LRS group showing the most significant changes.At the 24 h time point, there were significant differences among the three groups, and after 36 hours of perfusion preservation, the apoptosis level in the TMP-LRS group was lower than that in the HC-A group, but the difference was not significant.Conclusion The use of Lactate Ringer's solution containing Ligustrazine during organ procurement perfusion can achieve similar effects to those of HC-A solution, making it a potential substitute to reduce medical costs.
