目的 探討半乳糖凝集素3(Galectin-3)、細(xì)胞周期蛋白D1(Cyclin D1) 在甲狀腺細(xì)胞學(xué)Bethesda-Ⅲ類結(jié)節(jié)中的表達(dá)及臨床意義。 方法 對(duì)128例細(xì)胞學(xué)診斷為Bethesda-Ⅲ類結(jié)節(jié)的細(xì)針穿刺標(biāo)本進(jìn)行沉渣包埋,應(yīng)用Envision法檢測(cè)Galectin-3、Cyclin D1在標(biāo)本中的表達(dá)情況。 結(jié)果 組織病理確診為甲狀腺乳頭狀癌(papillary thyroid carcinoma,PTC) 68例,良性病變60例,包括結(jié)節(jié)性甲狀腺腫45例、濾泡性腺瘤11例、淋巴細(xì)胞甲狀腺炎4例。在PTC中Galectin-3、Cyclin D1表達(dá)明顯高于良性病變,陽(yáng)性率分別為92.6%和89.7%,兩者差異具有統(tǒng)計(jì)學(xué)意義(P<0.001)。兩者聯(lián)合檢測(cè)可以提高診斷的敏感性、特異性、準(zhǔn)確性,且在PTC與良性疾病中表達(dá)的差異具有統(tǒng)計(jì)學(xué)意義(P<0.001)。 結(jié)論 聯(lián)合檢測(cè)Galectin-3和Cyclin D1有助于甲狀腺細(xì)胞學(xué)Ⅲ類結(jié)節(jié)的診斷與鑒別診斷,為臨床治療提供可靠的依據(jù)。
Abstract:
Objective To investigate the expression and clinical significance of Galectin-3 and Cyclin D1 in Bethesda-Ⅲ nodules of thyroid cytology. Methods A total of 128 specimens of fine needle aspiration with Bethesda-Ⅲ nodules of thyroid cytology were carried out of sediment embedding slice, and the expression of Galectin-3 and Cyclin D1 in the specimens was detected by Envision method.Results There were 68 cases of papillary thyroid carcinoma (PTC), and 60 cases of benign lesions, including 45 cases of nodular goiter, 11 cases of follicular adenoma, and 4 cases of lymphocytic thyroiditis. The expression of Galectin-3 and Cyclin D1 in PTC was significantly higher than that in benign lesion. The positive rates of Galectin-3 and Cyclin D1 were 92.6% and 89.7% respectively and the difference was statistically significant (P<0.001). The sensitivity, specificity and accuracy of the diagnosis were improved by the combination of the two tests, and the difference of combined expression between PTC and benign disease was statistically significant (P<0.001). Conclusion The combined detection of Galectin-3 and Cyclin D1 is helpful for the diagnosis and differential diagnosis of Bethesda-Ⅲ nodules of thyroid cytology, and provides reliable basis for clinical treatment.
