目的 對比研究超微血管成像技術(shù)(SMI)、 能量多普勒超聲(PDUS) 及超聲造影(CEUS)在判斷類風(fēng)濕性關(guān)節(jié)炎活動性的價值。方法 對50例RA活動期患者進行超聲檢查,觀察SMI、PDUS、CEUS血流模式對最厚處滑膜血流的顯示率和滑膜血流分級的差異,并與DAS28、類風(fēng)濕因子(RF)、 C反應(yīng)蛋白(CRP)、 血沉(ESR) 進行相關(guān)性分析,評估SMI對RA患者關(guān)節(jié)炎癥活動性的臨床應(yīng)用價值。結(jié)果 在RA患者中,CEUS、SMI、PDUS對滑膜血流的顯示率分別為92%、88%、76%,SMI與CEUS較PDUS可以檢測出更多關(guān)節(jié)的滑膜血流,而SMI對關(guān)節(jié)滑膜血流的顯示與CEUS無明顯差異(P=0.159);通過評估滑膜血流分級發(fā)現(xiàn),SMI與CEUS,較PDUS,血流分級更高。CEUS、SMI及PDUS的滑膜血流分級與ESR、CRP、DAS28 均星正相關(guān)。結(jié)論 SMI在觀察RA患者增生滑膜血流信號方面優(yōu)于PDUS,可更準(zhǔn)確地評估RA患者的疾病活動性。
Abstract:
Objective To compare the value of superb microvascular imaging (SMI), power doppler imaging (PDUS) and contrast enhanced ultrasound (CEUS) in patients with activity of rheumatoid arthritis (RA). Methods The thickest synovium of 50 patients underwent superb SMI,PDUS and CEUS. The display rate of synovial blood flow and synovial blood flow classification were observed. The correlation between the DAS28,rheumatoid factor (RF), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR) and SMI were analyzed to evaluate the clinical application value of SMI in activity of RA. Results The blood flow detectable rate of CEUS, SMI and PDUS were 92%,88% and 76% respectively, and there was no significant difference in the detection of synovial blood flow between the SMI and CEUS (P=0.159). SMI and CEUS could detect more synovial blood flow, it had higher vascularity classification than PDUS. CEUS, SMI and PDUS were in good agreement with CRP,ESR and DAS28. Conclusion SMI is more sensitive in detecting synovial inflammation than PDUS in patients with RA. SMI can be a potentially useful imaging modality for accurately diagnosing and monitoring the disease activity of RA.
