摘要:
目的 探討公民逝世后器官捐獻供體的器官評估和功能維護的方法和效果。方法 回顧性分析我院重癥醫(yī)學(xué)科行器官功能支持維護的30例供體的臨床資料。30例捐獻者經(jīng)外院經(jīng)治醫(yī)生確定為潛在捐獻者后,被及時轉(zhuǎn)介給器官捐獻協(xié)調(diào)員,通過福建省衛(wèi)生健康委員會及紅十字會關(guān)于器官捐獻和潛在捐獻者身份識別轉(zhuǎn)診的規(guī)章制度,合法、合規(guī)地將捐獻者轉(zhuǎn)送我科進行器官維護,包括嚴(yán)格的肺保護、循環(huán)支持、精準(zhǔn)的液體管理和穩(wěn)定內(nèi)環(huán)境。捐獻者授權(quán)人簽署器官捐獻等相關(guān)文件,達到器官獲取標(biāo)準(zhǔn),由器官獲取團隊進行器官獲取。觀察供體器官獲取后的利用情況。結(jié)果 共獲得142個器官,成功用于移植。其中獲取心臟7例、腎臟56例、肝臟27例、雙側(cè)肺部共6例,雙側(cè)角膜共46例。移植后器官功能恢復(fù)良好,無死亡病例發(fā)生,2例肝移植發(fā)生早期移植物功能不良,6例腎移植后腎功能恢復(fù)延遲。結(jié)論 供體器官功能的評估與維持是公民逝世后器官捐獻的重要環(huán)節(jié),及時準(zhǔn)確地評估和維護供體器官功能是取得較好移植效果的關(guān)鍵因素。
Abstract:
Objective To explore the methods and effects of organ evaluation and functional maintenance of organ donors after the death of citizens.Methods The clinical data of 30 donors who received organ function support and maintenance in the department of critical care medicine in our hospital were analyzed retrospectively.After being identified as potential donors by doctors in other hospitals, 30 donors were referred to the organ donation coordinator in a timely manner.Through the rules and regulations of Fujian Provincial Health Commission and the Red Cross on organ donation and potential donor identification and referral, donors were legally and conformably transferred to our department for organ maintenance.It included strict lung protection, circulatory support, accurate fluid management and stable internal environment.The person authorized by donor signed organ donation and other relevant documents to meet the organ acquisition standards, and organ acquisition was carried out by the organ acquisition team.The utilization of donor organs was observed.Results A total of 142 organs were obtained and successfully used for transplantation.Heart(n=7), kidney(n=56), liver(n=27), bilateral lungs(n=6)and bilateral corneas(n=46)were obtained.The organ function recovered well after transplantation, no deaths occurred, early graft dysfunction occurred in two cases after liver transplantation,delayed recovery of renal function occurred in six cases after renal transplantation.Conclusion The evaluation and maintenance of donor organ function is an important part of organ donation after the death of citizens,timely and accurate evaluation and maintenance of donor organ function is the key factor to achieve a better effect of transplantation.
