摘要:
目的 探討無甲狀腺被膜接觸的甲狀腺乳頭狀微小癌(papillary thyroid microcarcinoma,PTMC)中央?yún)^(qū)淋巴結(jié)轉(zhuǎn)移(central lymph node metastasis,CLNM)的危險(xiǎn)因素。方法 回顧性分析354例于我院手術(shù)且術(shù)后病理證實(shí)為單灶性PTMC者的術(shù)前超聲和臨床病理特征與CLNM的關(guān)系。結(jié)果 單因素分析顯示,性別,年齡,癌灶的左右徑,前后徑,上下徑,體積,邊緣,微鈣化,距內(nèi)、外緣距離均與CLNM顯著相關(guān)。多因素分析顯示,男性,年齡<46歲,上下徑≥6.1 mm,微鈣化,邊緣不光整,癌灶距甲狀腺內(nèi)側(cè)緣≤2.4 mm和距外側(cè)緣≥8.0 mm是CLNM的獨(dú)立危險(xiǎn)因素,其中微鈣化的OR值最高(10.224)。結(jié)論 男性,年齡<46歲,上下徑≥6.1 mm,微鈣化,邊緣不光整,癌灶距甲狀腺內(nèi)側(cè)緣≤2.4 mm和距外側(cè)緣≥8.0 mm是無被膜接觸PTMC發(fā)生CLNM的獨(dú)立危險(xiǎn)因素,其中微鈣化風(fēng)險(xiǎn)值最大。
Abstract:
Objective To explore the risk factors of central lymph node metastasis(CLNM)of papillary thyroid microcarcinoma(PTMC)without thyroid capsule contact.Methods A retrospective analysis of the relationship between preoperative ultrasound and clinicopathological features of 354 patients who were operated in our hospital and proved to be unifocal PTMC by pathologyand CLNM.Results Univariate analysis showed that gender,age,transverse diameter(TD),anteroposterior diameter(APD),sagittal diameter(SD),volume,distance to inner-capsule(DTI),distance to outer-capsule(DTO),margin,and microcalcification were all significantly related to CLNM.Multivariate analysis showed that male,age<46 years,SD≥6.1 mm,microcalcification,ill-defined margin,DTI≤2.4 mm and DTO≥8.0 mm were independent risk factors for CLNM,the OR value of microcalcification was the highest(10.224).Conclusion Male,age<46 years,SD≥6.1 mm,microcalcification,ill-defined margin,DTI≤2.4 mm and DTO≥8.0 mm are independent risk factors for CLNM in PTMC without capsule contact,and microcalcification has the greatest risk.
