摘要:
目的 觀察白內(nèi)障患者術(shù)前Alpha角的分布特點(diǎn)、分析相關(guān)影響因素。方法 回顧性分析我院2019年白內(nèi)障患者204例(204眼),采用OPD-Scan Ⅲ測量患者Alpha角(LDist@angle)及明視Kappa角(PDist@angle)及瞳孔大小,使用OA-2000生物測量儀測量患者的眼軸、平均曲率、前房深度及晶體厚度。使用SIA矢量分析軟件繪制極坐標(biāo)散點(diǎn)圖,分析右眼及左眼LDist的分布情況; 使用獨(dú)立樣本t檢驗(yàn)分析不同性別、眼別間Alpha角的差異,使用Pearson相關(guān)分析其他生物測量參數(shù)對Alpha角大小的影響,線性回歸分析Kappa角與Alpha角的關(guān)系。結(jié)果 LDist平均值為(0.43±0.18)mm; 雙眼LDist多位于顳側(cè); 不同性別及眼別組間LDist比較,差異無統(tǒng)計學(xué)意義。LDist與眼軸、前房深度成負(fù)相關(guān),與年齡、角膜平均曲率、晶體厚度、瞳孔大小無明顯相關(guān)性。LDist與PDist大小呈線性正相關(guān)。結(jié)論 Alpha角分布比較集中穩(wěn)定; Alpha角大小與眼軸、前房深度成負(fù)相關(guān),當(dāng)無法測量Alpha角時,對于短眼軸、淺前房的患者可能需慎重使用多焦點(diǎn)人工晶體; 當(dāng)Alpha角>0.5 mm時,植入多焦點(diǎn)人工晶體術(shù)后發(fā)生光學(xué)干擾的可能性增加。
Abstract:
Objective To observe the distribution of kappa alpha angle before cataract surgery, analyze the relevant factors affecting it and the correlation between kappa angle and alpha angle. Methods A retrospective research was conducted.The preoperative angle alpha(LDist@angle),angle kappa(PDist@angle), and pupil size of 204 patients(204 eyes)were measured and recorded by OPD-Scan Ⅲ in 2019.The preoperative axial,mean corneal curvature,anterior chamber depth and lens thickness were measured by OA-2000.Independent-samplet tests were performed to test for significant differences in the magnitude of angle alpha preoperatively between genders and eyes.Pearson correlations were performed for correlation analysis of influence of parameters on alpha angle.Linear regression analysis was performed for the relationship between angle kappa and angle alpha. Results The mean value of LDist was(0.43 ±0.18)mm.There was no significant difference in LDist between different genders and different eyes.The distribution of LDist was mostly in the temporal side both in left and right eyes.LDist was negatively correlated with axial and anterior chamber depth.There is a linear positive correlation between PDist and LDist. Conclusion The distribution of angle alpha is centralized and stable.Age,anterior chamber depth and lens thickness have influence on and angle alpha.When angle alpha can not be measured, multifocal intraocular lens may be used cautiously for patients with short ocular axis and shallow anterior chamber.When angle alpha is greater than 0.5 mm, the possibility of optical interference after implantation of multifocal intraocular lens increases.
