摘要:
目的 探討經(jīng)陰道三維超聲多普勒(three-dimensional transvaginal ultrasound doppler, 3D-TVUD)技術(shù)在評(píng)估不明原因復(fù)發(fā)性流產(chǎn)(recurrent spontaneous abortion,RSA)患者子宮內(nèi)膜容受性(endometrial receptivity,ER)中的臨床應(yīng)用價(jià)值。方法 收集到我院就診的32例RSA患者及30例對(duì)照組,比較種植窗期兩組患者子宮動(dòng)脈及螺旋動(dòng)脈血流頻譜參數(shù)、子宮內(nèi)膜厚度、類型、容積和血管化血流參數(shù)如血管形成指數(shù)(VI)、血流指數(shù)(FI)、血管形成-血流指數(shù)(VFI)等各項(xiàng)參數(shù)的差異。結(jié)果 RSA患者子宮動(dòng)脈及螺旋動(dòng)脈血流阻力指數(shù)(RI)、搏動(dòng)指數(shù)(PI)均高于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05); RSA患者A型內(nèi)膜數(shù)、子宮內(nèi)膜厚度、容積、血管化血流參數(shù)(VI、FI、VFI)均小于對(duì)照組,差異有統(tǒng)計(jì)學(xué)意義(P<0.05); 而子宮動(dòng)脈收縮期峰值流速(Vmax)兩組間差異無統(tǒng)計(jì)學(xué)意義(P>0.05)。結(jié)論 3D-TVUD有助于評(píng)估不明原因RSA 患者子宮內(nèi)膜容受性,其參數(shù)可為臨床診斷和療效評(píng)估提供參考。
Abstract:
Objective To investigate the clinical value of three-dimensional transvaginal ultrasound doppler technique in detecting endometrial receptivity related ultrasound parameters in patients with unexplained recurrent abortion.Methods Thirty-two patients with RSA and thirty controls in our hospital were collected, and the differences of uterine artery and spiral artery blood flow spectrum parameters, endometrial thickness, a type, volume and vascularization blood flow parameters(VI, FI and VFI)in the two groups during the implantation window period were compared.Results The RI and PI of the uterine artery and spiral artery in patients with RSA were higher than that in control group, and the differences were statistically significant.The number of type A endometrium, endometrial thickness, volume, and vascularization parameters(VI, FI, VFI)of patients with RSA were all lower than those of the control group, and the differences were statistically significant.There was no significant statistical difference in uterine artery velocity between two groups.Conclusion 3D-TVUD is helpful for the assessment of endometrial receptivity in patients with unexplained RSA, and its parameters can provide reference for clinical diagnosis and efficacy evaluation.
