摘要:
目的 探討產(chǎn)前超聲與解剖鑄型在胎兒肺動(dòng)脈發(fā)育異常診斷中的應(yīng)用價(jià)值。方法 回顧性分析35例胎兒肺動(dòng)脈發(fā)育異常的產(chǎn)前超聲圖像,與解剖鑄型標(biāo)本對(duì)照,分析其特征及鑒別要點(diǎn)。結(jié)果 胎兒肺動(dòng)脈發(fā)育異常中,肺動(dòng)脈及其分支大小異常30例(肺動(dòng)脈狹窄18例,肺動(dòng)脈閉鎖9例,肺動(dòng)脈瓣缺如綜合征3例),肺動(dòng)脈起源異常5例(一側(cè)肺動(dòng)脈缺如2例,肺動(dòng)脈起自升主動(dòng)脈2例,肺動(dòng)脈吊帶1例)。引產(chǎn)后病理解剖25例,血管鑄型10例。超聲誤診3例:肺動(dòng)脈閉鎖誤診為共同動(dòng)脈干1例,肺動(dòng)脈閉鎖誤診為肺動(dòng)脈狹窄1例,室間隔缺損型肺動(dòng)脈閉鎖肺血來(lái)源誤診1例。結(jié)論 胎兒肺動(dòng)脈發(fā)育異常中肺動(dòng)脈閉鎖和狹窄較為常見(jiàn),產(chǎn)前超聲對(duì)胎兒肺動(dòng)脈發(fā)育異常具有重要診斷價(jià)值,解剖及鑄型的反饋有助于進(jìn)一步認(rèn)識(shí)胎兒肺動(dòng)脈發(fā)育異常。
Abstract:
Objective To investigate the value of antenatal ultrasound and vascular cast, autopsy in diagnosis of pulmonary artery dysplasia(hereinafter referred to as PAD).Methods Thirty-five fetuses with prenatal-ultrasound-diagnosed PAD were enrolled in this study.Ultrasound images were retrospectively processed for analysis, comparing with autopsy and vascular cast.Results Among the 35 fetuses with PAD, 30 of them were found dysplasia of pulmonary artery and its branches in pipe diameter(18 in stenosis, 9 in atresia, 3 in dilation), anomalous origin of pulmonary artery in 5 cases(absence of unilateral pulmonary artery in 2 cases, origin of pulmonary artery from ascending aorta in 2 cases, pulmonary artery sling in 1 case).Twenty-five cases underwent autopsy and 10 cases for vascular cast after termination.Three anomalies were misdiagnosed or missed diagnosed by ultrasound, one pulmonary atresia misdiagnosed as common arterial trunk, another one pulmonary atresia misdiagnosed as pulmonary stenosis, one blood supply of lung of pulmonary atresia with ventricular septal defect misdiagnosed.Conclusion Among the cases of PAD, pulmonary stenosis and atresia are most common.Antenatal ultrasound has significant diagnostic value in these cases.Feedback from autopsy and vascular cast is helpful for further understanding of PAD.
