摘要:
目的 探討腹腔鏡左肝外葉規(guī)則性切除治療左肝外葉膽管結(jié)石的臨床療效。方法 回顧性分析2004年1月至2019年1月我院收治的60例左肝外葉膽管結(jié)石患者臨床資料,分為實驗組(腹腔鏡手術(shù)組28例)和對照組(開腹手術(shù)組32例)。術(shù)前兩組患者年齡、性別、病程等臨床資料差異無統(tǒng)計學(xué)意義(P>0.05); 統(tǒng)計分析兩組手術(shù)時間、術(shù)中出血量、排氣時間、術(shù)后并發(fā)癥、住院時間、結(jié)石清除率、結(jié)石復(fù)發(fā)率、總體療效。結(jié)果 兩組手術(shù)時間、術(shù)中出血量、結(jié)石清除率、結(jié)石復(fù)發(fā)率、總體療效評估差異無統(tǒng)計學(xué)意義(P>0.05),但排氣時間、住院時間、術(shù)后并發(fā)癥差異有統(tǒng)計學(xué)意義(P<0.05)。結(jié)論 腹腔鏡規(guī)則性左肝外葉切除術(shù)治療左肝外葉結(jié)石安全可行且有效,具有微創(chuàng)、恢復(fù)快等優(yōu)點,值得推廣。
Abstract:
Objective To investigate the clinical effect of laparoscopic left extrahepatic lobectomy for the treatment of left external hepatolithiasis.Methods The clinical date of 60 patients with left external hepatolithiasis in our hospital from Jan.2004 to Jan.2019 were retrospective analyzed.The samples were divided into experimental group(28 patients for laparoscopic surgery)and control group(32 patients for open surgery).The patients in the two groups had no significant discrepancies in age, gender, course of disease and other clinical conditions(P<0.05).We used statistical analysis to evaluate the effect of treatment for the two group in time of operation, bleeding volume, time of exhaust, postoperative complications, hospital stays, rate of stone-free, rate of recurrence and overall effect.Results There were no significant statistical differences(P>0.05)between the two groups in time of operation, bleeding volume, rate of stone-free, rate of recurrence and overall effect.However, there were statistical differences(P<0.05)between the two groups in time of exhaust and hospital stays,postoperative complications.Conclusion It is safe and effective to use laparoscopic left extrahepatic lobectomy for the treatment of left external hepatolithiasis.The treatment has merits of minimal invasion, quick recovery and it is worthy of promotion.
