摘要:
目的 分析對比高頻超聲、X線和MRI在銀屑病關(guān)節(jié)炎手關(guān)節(jié)病變中的應用價值。 方法 選取有手關(guān)節(jié)病變的銀屑病關(guān)節(jié)炎患者30例,分別應用高頻超聲、X線及MRI對其進行觀察,分析比較3種方法的優(yōu)缺點。結(jié)果 超聲與MRI對銀屑病關(guān)節(jié)炎手關(guān)節(jié)病變(滑膜增厚、關(guān)節(jié)腔積液、骨質(zhì)破壞和血流信號增多)均有較高的檢出率,但兩者間差異無統(tǒng)計學意義(P>0.05)。MRI可檢查出骨髓水腫,對關(guān)節(jié)腔內(nèi)病變更有優(yōu)勢,而超聲側(cè)重于關(guān)節(jié)腔外的病變。在關(guān)節(jié)的骨質(zhì)破壞和新骨形成方面,X線與超聲和MRI比較有更高的檢出率(P<0.05)。結(jié)論 高頻超聲、X線和MRI在銀屑病關(guān)節(jié)炎手關(guān)節(jié)病變中的應用價值各有優(yōu)勢,可以相互補充。超聲和MRI較X線可以更加準確、全面地診斷病變,可成為銀屑病關(guān)節(jié)炎手關(guān)節(jié)病變診斷的首選方法。
Abstract:
Objective To compare the value of high frequency ultrasound, X-ray and MRI in evaluation of psoriatic arthritis with the hand joint involved.Methods A total of 30 cases of psoriatic arthritis with hand joint lesions were selected and observed respectively by high-frequency ultrasound, X-ray and MRI.The advantages and disadvantages of the three methods were analyzed and compared.Results In psoriatic arthritis, the difference of the detection rate of hand joint lesions(synovial thickening, synovial effusion, bone destruction and increased blood flow signal)between MRI and ultrasound was not statistically significant(P>0.05).MRI could detect bone marrow edema and was more advantageous for intra-articular lesions than ultrasound.However, ultrasound focused on lesions outside the articular cavity.Compared with ultrasound and MRI, X-ray had higher detection ratio of bone destruction and new bone formation of the joints(P<0.05).Conclusion High frequency ultrasound, X-ray and MRI, which are complementary, have their own advantages in evaluation of psoriatic arthritis with the hand joint involved.Ultrasound and MRI are more accurate and comprehensive than X-ray and can be the preferred methods for the diagnosis of hand joint lesions of psoriatic arthritis.
