摘要:
目的 探討血液灌流聯(lián)合腹腔穿刺引流治療高脂血癥型重癥胰腺炎的療效。方法 收集我科收治的伴有腹腔積液的高脂血癥型重癥胰腺炎患者42例,分為兩組,對(duì)照組24例,采 用傳統(tǒng)內(nèi)科治療; 觀察組18例,采用血液灌流聯(lián)合腹腔穿刺治療,比較兩組療效。結(jié)果 治療 后,除對(duì)照組C反應(yīng)蛋白(CRP)較治療前無明顯下降外,兩組血TG、TC、WBC、乳酸、淀粉酶等 均較治療前下降,觀察組上述指標(biāo)下降更明顯,差異有統(tǒng)計(jì)學(xué)意義(P<0.05); 觀察 組住院時(shí)間少于對(duì)照組,首次排氣時(shí)間早于對(duì)照組,胃腸引流量低于對(duì)照組,差異均有統(tǒng)計(jì)學(xué) 意義(均P<0.05)。結(jié)論 血液灌流聯(lián)合腹腔穿刺引流治療伴有腹腔積液的高脂血 癥型重癥胰腺炎患者可有效降低血脂,更快改善胃腸道癥狀,改善預(yù)后。
Abstract:
Objective Our study aims to compare the efficacy between the combined treatment of hemoperfusion with abdominal paracentesis drainage and conservative treatment for the hyperlipidemia type of severe pancreatitis with peritoneal effusion.Methods A total of 42 patients with the hyperlipidemia type of severe pancreatitis with peritoneal effusion were selected in the emergency department.Among them, 24 patients(control group)were treated with traditional medical treatment, and 18 patients(treatment group)were treated with hemoperfusion combined with abdominal paracentesis drainage.The curative effect of two groups of patients were compared.Results After treatment, except for C- reactive protein(CRP)in the control group, TG, TC, WBC, lactic acid and amylase in the two groups decreased significantly compared with those before treatment.However, each index in the treatment group decreased more significantly, and there were significant differences between the two groups (P<0.05).Moreover, compared with control group, the hospitalization time was shorter, the first exhaust time was earlier, and the gastrointestinal drainage was less in the treatment group(P<0.05).Conclusion During treatment of the hyperlipidemia type of severe pancreatitis with peritoneal effusion, the hemoperfusion combined with abdominal paracentesis drainage is more effective in reducing blood lipid, faster improving gastrointestinal symptoms, and ameliorating the prognosis.
