摘要:
目的 定量測肺泡巨噬細(xì)胞數(shù)量及肺泡巨噬細(xì)胞分泌白介素-1β水平,探討特發(fā)性肺泡蛋白沉積癥(PAP)患者肺泡巨噬細(xì)胞功能在其診斷及監(jiān)測中的意義。方法 連續(xù)收集呼吸科2010年4月至2017年12月就診的51例PAP患者。納入同期47例其他彌漫性間質(zhì)性肺疾病的住院患者作為對(duì)照組,所有患者收集肺泡灌洗液及血清標(biāo)本,通過血清巨噬細(xì)胞集落刺激因子抗體(GMAbs)水平檢測,進(jìn)一步明確PAP患者,以ELISA方法測定肺泡灌洗液巨噬細(xì)胞分泌白介素-1β水平,同時(shí)計(jì)數(shù)肺泡灌洗液巨噬細(xì)胞數(shù)量。結(jié)果 所有入選PAP患者均行血清巨噬細(xì)胞集落刺激因子抗體水平檢測,結(jié)果均呈陽性。患者肺泡灌洗液巨噬數(shù)量明顯低于對(duì)照組(P<0.01),但疾病不同嚴(yán)重程度兩組比較差異無統(tǒng)計(jì)學(xué)意義。肺泡巨噬細(xì)胞分泌IL-1β水平明顯低于對(duì)照組,差異具有統(tǒng)計(jì)學(xué)意義(P<0.01)。結(jié)論 PAP患者肺泡灌洗液巨噬細(xì)胞數(shù)量及肺泡巨噬細(xì)胞分泌IL-1β水平明顯低于對(duì)照組,間接反映了PAP患者巨噬細(xì)胞數(shù)量及功能明顯減低。
Abstract:
Objective The number of alveolar macrophages and the level of interleukin-1 beta secreted by alveolar macrophages were measured quantitatively to explore the significance of alveolar macrophage function in the diagnosis and monitoring of idiopathic pulmonary alveolar proteinosis(PAP). Methods Over seven years, 51 patients with PAP were diagnosed at our hospital and enrolled in the study. 47 patients with other diffuse interstitial lung diseases in the same period were included as control group. Alveolar lavage fluid and serum samples were collected from all patients. The level of serum macrophage colony stimulating factor antibody(GMAbs)was detected to further identify PAP patients. The level of interleukin-1 beta secreted by alveolar lavage fluid macrophages was determined by ELISA, and the number of alveolar lavage fluid macrophages was counted. Results All the selected patients with pulmonary alveolar proteinosis were tested for serum granulocyte macrophage colony stimulating factor autoantibody(GMAbs), and the results were positive. The number of macrophages in alveolar lavage fluid was significantly lower than that in the control group(P<0.01), but there was no significant difference between the two groups in different severity of the disease. The level of IL-1 beta secretion in alveolar macrophages was significantly lower than that in disease control group(P<0.01). Conclusion The number of macrophages in alveolar lavage fluid and the level of IL-1beta secreted by alveolar macrophages in PAP patients are significantly lower than those in the control group, which indirectly indicates that the number and function of macrophages in PAP patients decrease.
