目的 探討脊髓型頸椎病患者M(jìn)RI-T2WI信號(hào)強(qiáng)度(signal intensity, SI)改變與其臨床表現(xiàn)嚴(yán)重程度之間的關(guān)系。 方法 選擇我院就診的脊髓型頸椎病患者64例,均行MRI檢查,根據(jù)MRI-T2WI像上信號(hào)強(qiáng)度改變分為3級(jí),即0、1、2級(jí); 采用日本骨科學(xué)會(huì)評(píng)分(JOA)評(píng)估患者的脊髓功能狀態(tài); 采用頸椎功能障礙指數(shù)量表(NDI)評(píng)估患者的頸椎失能程度。結(jié)果 0級(jí)15例,1級(jí)34例,2級(jí)15例,3組病程差異有統(tǒng)計(jì)學(xué)意義(P=0.020 5),JOA和NDI評(píng)分差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。 結(jié)論 脊髓型頸椎病患者M(jìn)RI-T2WI像信號(hào)強(qiáng)度改變與癥狀持續(xù)時(shí)間有關(guān),病程越長(zhǎng),信號(hào)強(qiáng)度越強(qiáng),而單獨(dú)的MRI-T2WI信號(hào)強(qiáng)度分級(jí)不能反映患者的臨床癥狀嚴(yán)重程度。
Abstract:
Objective To explore the correlation between intramedullary cord signal intensity(SI)changes on T2-weighted MRI and the severity of clinical symptoms in patients with cervical spondylotic myelopathy(CSM). Methods Sixty-four patients with CSM who visited to the outpatient clinic of our department were enrolled in this study. All patients underwent MRI scan and were classified into three grades: grade 0, grade 1 and grade 2. The Japanese Orthopedics Association(JOA)and the Neck Disability Index(NDI)were used to assess the function of the spinal cord and cervical spine, respectively. Results Of the 64 CSM patients, 15 were classified as grade 0, 34 as grade 1 and 15 as grade 2. There were significant differences of the duration of symptoms among the three groups(P=0.020 5). No significant differences were found both the JOA and NDI scores among the three groups(P>0.05).Conclusion The SI changes of MRI-T2WI images are related to the duration of symptoms in patients with CSM, and the patients with longer symptom durations had high grades of intramedullary cord T2-weighted MRI SI changes. However, MRI T2 image classification alone should not be considered in isolation when assessing severity of clinical symptoms in patients with CSM.
