摘要:
目的探討圍手術(shù)期預(yù)防性使用不同的藥物干預(yù)方式對(duì)乳腺區(qū)段切除術(shù)后惡心嘔吐(PONV)發(fā)生情況的影響,為短小手術(shù)預(yù)防PONV藥物干預(yù)方案提供一定理論依據(jù).方法選取復(fù)合納入標(biāo)準(zhǔn)的患者90例.將患者隨機(jī)分為Ⅰ組(地塞米松組)、Ⅱ組(托烷司瓊組)、Ⅲ組(地塞米松和托烷司瓊聯(lián)合用藥組)3組,每組30例.在麻醉誘導(dǎo)后,Ⅰ組靜脈注射地塞米松,Ⅱ組靜脈注射托烷司瓊,Ⅲ組靜脈聯(lián)合注射地塞米松和托烷司瓊,觀察和記錄術(shù)后24h患者發(fā)生PONV的患者數(shù)量,對(duì)實(shí)驗(yàn)數(shù)據(jù)統(tǒng)計(jì)分析,得出結(jié)果.結(jié)果3組患者中,Ⅲ組PONV等級(jí)(WHO標(biāo)準(zhǔn))和術(shù)后PONV視覺(jué)模擬評(píng)分(VAS)均低于Ⅰ組和Ⅱ組,差異具有統(tǒng)計(jì)學(xué)意義(P<0.05).結(jié)論與單獨(dú)靜脈注射地塞米松或托烷司瓊相比較,靜脈聯(lián)合注射地塞米松和托烷司瓊對(duì)預(yù)防乳腺區(qū)段切除術(shù)后患者惡心嘔吐效果更優(yōu).
Abstract:
Objective To assess the feasibility of using the different medicines:dexamethasone,tropisetron and dexamethasone combined with tropisetron on postoperative nausea and vomiting(PONV) after segmental mastectomy.Methods Ninety patients with the standard of intake were randomly divided into three groups:Ⅰ Group (dexamethasone group),Ⅱ Group (tropane group) and Ⅲ Group (dexamethasone & tropisetron group) respectively,with 30 cases in each group.The patients were treated with dexamethasone,tropisetron and dexamethasone combined with tropisetron after anesthesia induction.Observation of PONV in the three groups within 24 hours was recorded.Results The Ⅲ Group had the least grade (WHO) of PONV,and the least scores (VAS) in the three groups (P<0.05).Conclusion Dexamethasone combined with tropisetron can significantly decrease PONV better than the single administration of dexamethasone or tropisetron.
